Targeted case management (TCM)

CHOICES offers Targeted Case Management to many different counties in North and Central Florida. We help children, young adults, and families live successful and healthy lives.


Our Targeted Case Management service is child and family driven. We understand that each person faces very different challenges and we want to work with them to create a plan of action that targets their specific, individual needs.


Our Targeted Case Managers (TCM's) are community advocates who identify individual strengths and needs, the help to develop a plan to address those needs. The goal of our Targeted Case Managers is to provide help for youth and adults whose natural support system or dependency case management ends. TCM's help determine the appropriate services available in the community. They actively assist their clients to obtain those benefits then help them to maintain the services.


What Is Targeted Case Management?

Targeted Case Management is a child- and family-driven and community-based service. Our Targeted Case Managers are community advocates who will interview you to identify your strengths and needs, then come up with a plan to address those needs. They will help determine the appropriate services available in your community to address those needs. Then advocate for you to receive those benefits and help you to maintain those services.


Who Benefits From Targeted Case Management?

We serve children and young adults between the ages 0 to 23 and families if they meet the following criteria:


  1. The person/family has a mental health disability and needs someone to help them coordinate and maintain services to ensure they keep/better their living conditions.
  2. The person/family needs help with learning to be independent in their education, employment and social environments.
  3. If he/she needs a support system in order to access needed medical and social benefits of their choice.


 CHOICES House is a 501(C)3 Non-Profit Organization. We are thankful for the support of local businesses as well as private contributions. We are always looking for opportunities to be of service to our community.
